Queen Of The Otaku: There Can Only Be One takes you through a common route with 3-5 choices, depending on the route you take. There are three possible endings.
This guide will tell you which choices to make to access the three endings, a missable CG variation, and explains the situation regarding the adult patch.
The Steam version of Queen Of The Otaku: There Can Only Be One does not include Steam Achievements, which is standard for Shiravune visual novels.

18+ Content
If you have purchased the Steam version of Queen Of The Otaku: There Can Only Be One, you can download the free 18+ DLC via Johren. You will need to register an account first. Then download the patch, unzip the files, and copy them into Queen Of The Otaku: There Can Only Be One’s install folder.
Either playing the Johren version or the Steam version with this patch is required to access any of the endings. The Steam version without the patch removes large parts of the game, including the endings. There is no context for several of the choices and some are missing.
While 18+ content is included with the patch or adult version, mosaic censorship remains in place.
